Trust me, you're in good hands. 💫✨ My salon opeprates with 15 minute grace period, after that time you are considered a no-show and your appointment will be canceled and all unpaid fees will be due before being able to rebook. >>Minor Piercing Requirements As Required By State Law<<< A: Parent Present with their valid government issued Picture ID. Minor with a valid government issued picture ID with matching address and last name as parent. If either are different or missing, then we require a birth certificate or court issued papers showing guardianship. OR A: Parent Present with their valid government issued Picture ID. Minor with photo ID and Minor’s birth certificate showing age and guardianship (Court issued documents like divorce papers will work in place of birth certificate). Forms of ID that are applicable are as follows – school ID, school yearbook within the last 2 years, passport. Step parents, siblings, grandparents, and anyone else can not sign for a piercing unless we have court documents or a notarized paper signed by the parents stating that they are the legal guardian.
CANCELLATION POLICY
48 Hour Cancellation Policy and Nonrefundable Deposit I, [Client's Full Name], acknowledge and agree to the following cancellation policy and nonrefundable deposit terms set forth by PiercingBeauty by Tabitha ("Company"): 1. Cancellation Policy: I understand that I must provide a minimum of 48 hours' notice for any cancellations or rescheduling of appointments. Failure to do so will result in a cancellation fee, as outlined below. 2. Nonrefundable Deposit: I agree to pay a nonrefundable deposit at the time of booking to secure my appointment. This deposit will be deducted from the total cost of the service. If I choose to cancel my appointment withing 48 hours of my scheduled time, the deposit will not be refunded. 3. Rescheduling: In the event that I need to reschedule my appointment, I may do so once, subject to availability, without incurring any additional charges. However, if I choose to cancel or reschedule a second time, the initial deposit will be forfeited. 4. Cancellation Fee: If I cancel my appointment within 48 hours of the scheduled appointment time, I understand that I will be charged a cancellation fee equal to 50% of the total cost of the service. This fee will be deducted from the deposit I have already paid, and if the deposit is insufficient to cover the cancellation fee, I will be responsible for paying the remaining balance. I acknowledge that the Company has set these policies to ensure efficient scheduling and to compensate for any lost business opportunities resulting from last-minute cancellations or rescheduling. I understand and accept the financial obligations associated with these policies. I have read and fully understand the 48 Hour Cancellation Policy and Nonrefundable Deposit terms as outlined above. I agree to abide by these terms and acknowledge that the Company reserves the right to modify or waive these policies at its discretion.
